Beginning on Dec. 1, 2023, NC Medicaid will cover people ages 19 through 64 years with higher incomes. You may be able to get health care coverage through Medicaid even if you didn’t qualify before.
Medicaid pays for doctor visits, yearly check-ups, emergency care, mental health and more – at little or no cost to you.
How to apply for Medicaid:

How will you know if you can get full health coverage through NC Medicaid?
If you already receive limited benefits through Family Planning Medicaid (if not continue to next section), you may now be eligible for full health coverage through Medicaid.
- You will get a letter from your local Department of Social Services letting you know that you will start getting full Medicaid coverage.
- You will then be assigned a health plan, and your health plan will send you a packet in the mail. It will include a new Medicaid ID card (if you want to change your health plan, you have 90 days to pick a new one).
- Your ID card also has the name of your primary care provider.

If you do not have Family Planning Medicaid, you may still qualify for full health coverage through Medicaid.
Most people will be able to get health care coverage through Medicaid if they meet the criteria below.
- You live in North Carolina
- Age 19 through 64
- You are a citizen (some non-U.S. citizens can also get health coverage through Medicaid)

Medical Provider Changes:
If you are approved for Medicaid and receive an insurance ID card with a different primary care provider listed, you have the option to change your provider easily!
You can choose one of these three ways:
- Contact your insurance plan by calling the number on the back of the insurance card.
- Call the Medicaid Enrollment Broker at 833-870-5500.
- Choose the Provider Change Form from the list below and fax it to the number provided on the form. OR, contact HealthNet Gaston and we can help to guide you!
