Our Programs

HealthNet Gaston supports the health and social needs of underserved community members through a comprehensive range of programs designed to promote lasting wellbeing. From our Rapid Rehousing ESG Program and Street Outreach efforts that connect individuals experiencing homelessness with safe, permanent housing, to our Justice-Involved Transition Project supporting incarcerated individuals reentering the community, we address critical barriers to stability through a Housing First approach.

As the lead agency for the NC-509 Continuum of Care, we coordinate regional efforts across Gaston, Lincoln, and Cleveland counties, while also fostering systems change through the Western NC Youth Homelessness Partnership for young people up to age 24. Additionally, our SHIIP Coordinator offers free, unbiased Medicare and Medicaid counseling to help individuals navigate their healthcare options with confidence. Through education, advocacy, and essential resources, HealthNet Gaston creates pathways to housing stability, health access, and long-term wellbeing for those who need it most.

Rapid Rehousing ESG Program

HealthNet Gaston’s Rapid Rehousing program helps individuals and families experiencing homelessness quickly secure safe, permanent housing. Through short-term rental assistance, housing navigation, and personalized case management, we remove barriers and support long-term stability using a Housing First approach.

Justice-Involved Transition Project

In partnership with Kintegra Health, HealthNet Gaston’s Justice-Involved Transition Project supports incarcerated individuals as they re-enter the community. We provide housing assistance, case management, and connections to healthcare and behavioral health services, helping participants achieve stability and reduce recidivism.

Street Outreach Program

HealthNet Gaston’s Street Outreach Program connects with individuals experiencing homelessness where they are—on the streets, in encampments, and in other unsheltered locations. Our team provides immediate support, essential resources, and pathways to housing and services, building trust and promoting long-term stability.

Western NC Youth Homelessness Partnership

Creating pathways to stability for youth up to age 24 who are experiencing, have experienced, or are at risk of homelessness, the Western NC Youth Homelessness Partnership is a collaborative coalition committed to ending youth homelessness across the rural regions of Western North Carolina. By uniting community organizations, local governments, and dedicated advocates, we're redesigning the system to ensure every young person has access to safe housing, supportive services, and the resources needed to build a secure and promising future.

HealthNet Gaston

SHIIP & Medicare/Medicaid Assistance

HealthNet Gaston proudly employs the Gaston County SHIIP Coordinator to help individuals navigate Medicare and Medicaid with confidence. Through the Senior Health Insurance Information Program (SHIIP), we provide free, unbiased counseling on health insurance options, prescription drug plans, and cost-saving resources. Our goal is to ensure every client understands their benefits and makes informed decisions for their healthcare needs.

Continuum of Care NC-509 Lead Agency

HealthNet Gaston serves as the Lead Agency for the NC-509 Continuum of Care, coordinating regional efforts to prevent and end homelessness across Gaston, Lincoln, and Cleveland counties. We bring together service providers, local governments, and community partners to develop strategies, secure funding, and implement programs that create pathways to housing stability for individuals and families in need.
